Frequently Asked Questions

To “Register” simply select the “Register Now” button on the home page and follow the instructions on the page. If you were previously enrolled in Shamir’s RCPV Rewards Program and already have a W-9 on file, then complete the required Registration section information. If this is your first time joining Shamir’s RCPV Rewards program, follow the full Registration process, we will also need a completed W-9 to be faxed to our offices at 858-444-3842.

Your practice must sell a minimum of 15 pairs of qualified Shamir lenses per month to accrue points.

*Points will be given for Glacier Plus™, however, sales will not count toward the 15 pair minimum.

**No points will be given for Shamir Duo™ and Shamir Element™. Sales will count towards 15 pair minimum only.

Why am I required to Enroll? All practices, new to the program, are required to enroll to properly set-up their Account with the required information: practice name, contacts, point recipients and laboratory along with a W-9 for tax purposes. If this is your first time joining Shamir’s RCPV Rewards program, follow the full Registration process, we will also need a completed W-9 to be faxed to our offices at 858-444-3842.
